SData query with multiple parameters

We have not been successful getting an SData query to function with more than one parameter, unless we use "or".  We tried parameter1 and parameter2, and that didn't work.  We can get parameter1 or parameter2 to function properly.  However, parameter1 or parameter2 or parameter3 will not work. 

 

Here's the resulting SData query that we're assembling:

http://crm-srv1-test:3333/sdata/slx/dynamic/-/contacts?where=FirstName eq 'John' and LastName eq 'Smith' and type eq 'alumni'

 

Our code is attached.  Can someone shed some light on this?  Are we using the wrong syntax?

Capitalize the "T" in "type".
